As case managers we are expected to continue our
professional growth throughout our careers.
One way to enhance that growth is to become involved in a national
organization of multidisciplinary healthcare leaders, led by CMSA, who are
committed to improving the quality of transitions of care. The National Transition of Care Coalition
(NTOCC) was established specifically for this purpose.
NTOCC’s mission is to raise awareness about
transitions of care among health care professionals, government leaders and
consumers to increase the quality of care, reduce medication errors and enhance
clinical outcomes. To this end, NTOCC
develops consensus regarding recommended actions that can be taken by all
participants in the healthcare system to improve transitions of care. NTOCC strives to provide a method of
communication to consumers for information when choosing healthcare options and
also serve as a clearinghouse for tools and intervention resources to support
providers and consumers to achieve safer and better transitions.
NTOCC Associate Membership is available to anyone
interested in improving the quality of care transitions. And provides downloadable tools that can be
shared with your clients, peers and other healthcare team members from any area
of practice. These tools are available
at no cost from their website. And
provide the case manager with excellent opportunities for client advocacy.
