Lifetime Achievement in Case Management Award

The Chapter Board of Directors presents the Lifetime Achievement in Case Management Award to the individual who has demonstrated throughout their case management career the highest level of professionalism while elevating the practice of case management to a higher level.  This individual has acted as a mentor for other case managers by example and through willingly imparting her/his knowledge and experience to others in a mission to raise the awareness of the impact that case management can have on health care outcomes. 

Criteria for the Lifetime Achievement in Case Management Award

  • Current member of DFW CMSA in good standing
  • Minimum of twenty (20) years case management experience
  • Documented examples of contributions made to the practice of  case management
  • Documented examples of mentoring other case managers
  • Documented service to the DFW CMSA Chapter
  • Nomination by a current BOD member to the BOD for consideration and approval
